Register with Health Care Connect
Ontario’s free Health Care Connect program has a Care Connector search for a family doctor, nurse practitioner or primary care team accepting patients in your community, including Old Toronto. You need a valid Ontario health card, and the mailing address must match your health card. Register online or call 811 (TTY 711), 24/7. Urgent needs are matched first; registering does not guarantee a quick match, so keep calling clinics too. Full Health Care Connect guide →
